Dear Applicant,

Drew’s Team’s mission is focused on reimbursing families for their CF related expenses to lesson the burden of this costly disease. In order to process your application in the most efficient manner, Drew’s Team requires a fully completed application.

Prior to filling out your application, gather any documentation regarding the CF-related expenses that you’ve had in the last 12 months for which you are requesting reimbursement (i.e. Explanation of Benefits, Prescription Receipts, Medical Facility Parking Receipts, etc). Expenses should be directly related to you or your child’s CF treatment or health and well being.

Enter all of the information requested into the online pdf application form THAT YOU CAN DOWNLOAD BY CLICKING THE BUTTON AT TOP. All fields are required for proper review of your grant request. Then, print your application or save it as a document on your desktop. Documentation of your CF-related expenses is required. Failure to submit proof of expenses will result in those expenses not being considered for reimbursement.

Next, print and sign the HIPPA release form to allow verification of medical history. If you are being referred by Boston Children’s Hospital, your social worker will have a HIPPA release form to sign and will attach to your application.

Finally, fax your printed application and applicable documentation to Drew’s Team at 603-912-5946. Drew’s Team will also accept emailed applications and scanned copies of your expense documentation. Please email copies to grantapplication@drewsteam.org.

Please understand that Drew’s Team is a non-profit organization comprised solely of volunteers looking to make a difference for families affected by CF. To ensure thorough review of your application, please allow 7-10 business days for your response. Applications are processed in the
order in which they are received.
